Good dental health is important to the overall health of you and your family. Health Choice Integrated Care covers emergency, preventative and routine dental services for members under the age of 21.
Young adults 18, 19 and 20 years of age should have a dental check-up every six months. You do not need a referral from your doctor to see a general dentist.
To find a dentist, please see the Health Choice Integrated Care Provider Directory. You can also call Member Services, Monday – Friday, 8 a.m. – 5 p.m. at 1-800-640-2123. We will help you find a dentist.
Dental health services for members ages 21 and older are limited to services that are related to a medical condition. Services are limited to a dental examination, dental x-ray and medication for a medical condition like acute pain, infection or fracture of the jaw.
Health Choice also covers limited dental services as needed for a member who is about to receive an AHCCCS covered organ transplant and for members getting ready for radiation treatment for certain cancers.
